Gustavus COVID-19 Self-Reporting Form Posted on April 15th, 2020 by

Dear Gustavus students, faculty, and staff,

With the extension of the Minnesota Governor’s Stay at Home Order and with models predicting the likely peak for COVID-19 spread to be well into the summer months, Gustavus has created a form for all students and employees who choose to notify the College in the event that they have COVID-19 symptoms, diagnosis, or exposure to COVID-19.

Individuals who choose to self-report by filling out this form: COVID-19 Self-Reporting Form will receive follow-up communication from either the Dean of Students Office (for students) or the Office of Human Resources (for employees) outlining assistance available to you from the College. Each member of the Gustavus community can decide for themselves if they wish to share this information with the College.

Individuals who report that they have been on campus within the past 14 days or in close contact with someone who has been on campus within the past 14 days will also receive a communication from the Gustavus Health Service with the goal of assessing campus COVID-19 exposure risk.
